Please be mindful and arrive on your scheduled time. You must complete and sign the required intake / consent forms before receiving any service.

 

CANCELLATION

Please understand we have reserved an appointment especially for you. We would appreciate your cooperation in regards to cancellation. If you've scheduled an appointment and find you must cancel, please inform us MINIMUM 24 HOURS prior to your scheduled time, otherwise BOOKED SERVICE will be charged in *FULL PRICE.
